In consideration of and as introduction to my enrollment as a student of VIDA YOGA (VIDA WELLNESS CORP.) (a) BGC: Arya Residences Condominum, Tower 1, McKinley Parkway, Bonifacio Global City, Fort Bonifacio, City of Taquiq; and/or (b) ALABANG: 2F Commercenter, Commerce Ave, Filinvest, Muntinlupa City, Metro Manila:

1. PHYSICAL HEALTH. I have been examined by a licensed physician within the past six (6) months and I have been found by such physician to be in a good physical health and fully able to perform all yoga exercise(s) which I am to learn and perform during classes and workshops at VIDA YOGA;
    

2. INSTRUCTION(S). I will faithfully follow all instructions given me by VIDA YOGA and its instructors as to when, where and how to perform and not to perform yoga exercise(s), it being understood that any deviation by me from such instructions shall be at my own risk;
    

3. LIABILITY. I will not hold VIDA YOGA, its instructors and employees responsible for any injuries suffered, accident, or physical impairment and I release VIDA YOGA, its instructors and employees from any and all liability, claims, demands, actions related to, arising out of, or are in any way connected with my participation in the classes and workshops at VIDA YOGA, including those allegedly attributed to the negligent acts of omission;
    

4. STANDARD OF CARE. I understand and I know that I am to receive instructions in yoga theory and exercise(s) only, and I will not hold VIDA YOGA and its instructors or employees to any higher standard of care than that applicable to the school of yoga theory and exercise; and
    

5. FEES. Any amount paid herewith, incl. registration fees, are non-refundable. Any refundsshall be entirely within the discretion of VIDA YOGA.
